Below is the MCLRASL Game Report Form.
Please fill out the form below by TABBING through and selecting/inserting/replacing text in each box.
Please submit within 24 hours of completion of your games. Each CR will submit 1 form for EACH game they CR'd. DO NOT Submit a form if you were an AR!
All RED CARDS need to be called in and possible USSF Report completed depending on the circumstances.
BE SURE ALL FIELDS ARE ENTERED BEFORE SUBMITTING.
